Patient-centered care refers to a system of health care that establishes a partnership between providers and patients, ensures that care decisions respect patients’ needs and preferences and provides the support patients need to make informed decisions and participate in their own care. Through this model, our Primary Care Provider teams develop ongoing relationships with their patients that lead to trust and open communication. This improves the level of care patients receive.
